REPEAT PRESCRIPTIONS must be authorised by your doctor.  You may order prescriptions by handing in the left-hand side of your Prescription Form, OR by telephoning the Prescription Line weekdays between 9am - 10am and 1:30pm - 3pm on 01383 416661, OR by ordering on-line from this page.

 

Please ensure that you order your prescription at least two working days before you require the medication.

 

Instructions for ordering a Repeat Prescription online

To ensure confidentiality you need your "Patient ID Number" before ordering your medication on-line.  This is printed at the top of the left-hand page of your prescription.
Please note that this is different from the PIN that you may have been given when ordering on-line began.

In the boxes below please fill in the Patient ID number and your date of birth.

Use the boxes to provide the name(s) of the medication(s) and dose i.e 5mg; 10mg etc (as on the container or the prescriptions slip) that you would like re-prescribed. Each box can take over 200 characters.

 

If you wish to collect your prescription direct from the Pharmacy please enter the name of the collecting pharmacy.  Please provide any other information you feel is relevant in the box marked 'comments'.

 

If you wish you may print a copy of your order by clicking on 'printable version' at the foot of the page

Please remember to click on 'submit' at the foot of the page after you have filled out the form.  You should recieve a brief acknowldgement if you do not your request may not have been submitted correctly.
