If you wish to register for Patient Access which is run by our clinical system supplier EMIS and links to our clinical system please complete the form and hand into reception. There are currently no advantages to this service as nearly all the functions are switched off and we are not recommending its use to our patients. Repeat prescriptions can be requested through this portal however the repeat prescription form on this website is more reliable and flexible.

If you have been asked by the practice to complete an HRT review form please complete the attached form and hand in to the reception desk. Blank forms are also available from reception.The form requires blood pressure readings, if you cannot read your blood pressure an appointment should be made with a healthcare assistant to have your BP checked.
